Admissions / Referrals
People who are over the age of 18, are medically stable and have sustained a brain injury are able to apply for services at CCRP. We provide the necessary supports and supervision to ensure a client's safety and well-being. Our program is able to provide services to individuals with cognitive, physical, emotional, and behavioral issues stemming from:
- Traumatic Brain Injuries
- CNS Infections
- Aneurysms
- Anoxia and Hypoxia
- Brain Tumors
- Exposure to Toxins
- Strokes
- Drug/Alcohol Abuse
In order for Cortland Community Reentry Program to determine appropriateness for admission to our program, we will need medical records regarding the injury and current physical and medical status. Once a referral has been determined appropriate and funding has been approved, Cortland Community Reentry Program can begin the admission process which will include finding appropriate living arrangements, assigning a Case Manager/Service Coordinator, and obtaining the necessary information that will meet the needs of each individual. We are able to accept payment through insurance companies, private pay agreements, and through the NYS Department of Health Medicaid Waiver Program. We will provide any needed assistance in determining and securing appropriate funding.
