Services Provided
CASE MANAGEMENT / SERVICE COORDINATION
Case Management/Service Coordination responsibilities include the development and implementation of a comprehensive treatment plan with members of the treatment team, ensuring the participant has a safe and secure residence in the community, revising the treatment plan on an ongoing basis, and encouraging the participant to increase his/her ability to problem solve and work toward an independent life style. Additionally, this service coordinator is responsible for ensuring coordination, communication, and cooperation between all the treatment providers.
INDEPENDENT LIVING SKILLS TRAINING
Independent Living Skills (ILS) are services individually designed to improve the ability of a participant to engage in functional tasks in the home as well as the community. These services include self-care, medication management, task completion, communication skills, money management skills and ability to maintain a household.

INTENSIVE BEHAVIOR PROGRAMS
Intensive Behavioral Programs (IBP) are individual interventions designed to decrease the participant’s severe maladaptive behaviors, which, if not modified will interfere with the individual’s ability to remain integrated in the community. Behavioral programs consist of a comprehensive assessment of maladaptive behaviors; development of a structured behavioral intervention plan, which has as its primary focus the teaching of socially appropriate behaviors and elimination of maladaptive behaviors; implementation of the plan and ongoing training to all services providers.
COMMUNITY INTEGRATION COUNSELING
Community Integration Counseling (CIC) is an individual service designed to assist the participant to more effectively manage and overcome the difficulties and stresses confronted by an individual with traumatic brain injury living in the community. This service is provided in the residences, the community or at the clinic building. We also offer this service to the participant’s family if they have a significant role in supporting the participant.
PHYSICAL FITNESS PROGRAM
Often individuals discharged from hospitals or other rehabilitation facilities are lacking in physical strength and endurance. Others may need continued physical or occupational therapy to increase their functional capacity. These needs are met in our physical fitness program.
ART THERAPY
STRUCTURED DAY PROGRAM
The Structured Day Program (SDP) includes services provided in an outpatient congregate setting to assist an individual with social skills enhancement and skill development. The program is typically provided in a group format and includes such activities as physical function, memory skills, alcohol and substance abuse education, life skills, men and women’s issues, relational dynamics and current events. Additionally they include social outings such as picnics, sporting events, antique car shows, and other regional outings.
FAMILY SERVICES
Cortland Community Reentry Program recognizes the importance of family involvement in the rehabilitation process. We hope to encourage your involvement by offering the following services for your convenience:
- Easy access location: off Interstate 81, 30 miles south of Syracuse, Syracuse airport and AMTRAK.
- Reduced rates on motel accommodations within two miles of the facility.
- Activities and meetings scheduled to encourage family participation during weekends and holidays.
- Opportunities to participate in actual therapy sessions on a regular or periodic basis.
- Family counseling available, as needed, for education about brain injury, counseling, or family therapy. Support group meetings with other family members.
- A resource library for families including articles, newsletters, and videotapes on brain injury rehabilitation.

- Teleconference system allowing you to phone in toll-free for important team and family meetings at times when you are unable to be present.
- Staff members work on the weekends and are available to meet with you if you are unable to visit during the week.
- Assistance in identifying resources in the community, in preparation for discharge.
- Follow-up interviews are conducted periodically after discharge to confirm that the transition from rehabilitation has been successful and to assist with any unexpected difficulties or needs.
RECREATIONAL OPPORTUNITIES
As important as hard work is, recreational outings are an important part of a community reentry program. CCRP provides consumers an opportunity for seasonal activities and Other outings in the community.
